Brief Summary
Under the direction of the Chief Medical Officer, the Director of Quality will manage, track, and evaluate all Center performance measures, identify data trends and make recommendations for new health center clinical and operational quality improvement initiatives when appropriate. Quality Director will participate in and assist in preparation of clinical audits and surveys.

Requirements/Qualifications
Practical knowledge of tools and techniques of continuous Quality Improvement and proficient in EMR, MS Office required.  Must be familiar with State & Federal law, HRSA, Joint Commission and PCMH accreditation. Master’s Degree in Healthcare, Nursing from an accredited College or University.  3-5 years of performance improvement, risk management, and quality leadership experience at director level or experience in related healthcare field, preferably in ambulatory care setting. CPHQ or Lean Six Sigma Certification required. Must be knowledgeable in The Joint Commission and NCQA PCMH accreditation and certification standards.

Brief Summary
Under the direction of the Chief Medical Officer, Director of Nursing will oversee standards of nursing practice.  DON will coordinate clinical support services required for timely and effective service delivery to patients in a Patient-Centered Medical Home (PCMH) model of care.  Responsible for knowledge of and compliance, with regulatory or credentialing agencies, such as Joint Commission, state and federal agencies.

Requirements/Qualifications
Bachelor’s Degree in Nursing required or 3-5 years clinical management experience, preferably in ambulatory setting. Current NJ RN licensure & BLS certification.

Requirements/Qualifications
Graduate of an accredited School of Dentistry.  3-5 experience in general dentistry. Maintenance of Continuing Educational Requirement to maintain licensure. OB/GYN RN Case Manager

Brief Summary
Under the supervision of Nurse Manager, the OB/GYN Case Manager will provide case management and health education services to all prenatal patients.  Case Manager completes prenatal intakes, obtains insurance authorizations and assists patient with referrals to community services.  Ability to relate and communicate well with diverse population.

Requirements/Qualifications
Graduate of accredited RN Nursing Program.  Associates or Bachelor’s Degree in Nursing required. Current NJ licensure and BLS certification. 3-5 years nursing experience, preferably in OB/GYN.  Strong interpersonal, communication and organizational skills.  Proficient in computers and EMR software.  Ability to relate and communicate well with diverse population. Brief Summary
Under the supervision of the Care Coordination RN and Primary Care Director, the Care Management LPN will be responsible for the clinical case management of and outreach to patients that are a part of CAMcare’s Medical Home Savings Program.  They will develop outcomes based strategies to proactively engage UHC, Horizon, & other CAMcare patients as assigned.  The Care Management LPN will collaborate with health specialists to determine patient health needs and the availability of services, and to develop goals & action plans for meeting those needs.  

Requirements/Qualifications
Graduation from an accredited LPN program with two or more years of hospital or public health nursing experience.  Current and valid NJ Practical Nurse licensure and BLS certification.  At least one year of experience in each of the following disciplines; Internal Medicine, Pediatrics or OB/GYN.  Daily access to reliable transportation for home, hospital, and/or other community service visits. Proficient in MS Office and EMR software. Ability to relate and communicate well with diverse population.  Bilingual (English/Spanish) a plus.


Certified Medical Assistant

Brief Summary
Under the direction of RN, the CMA will room patients, take vital signs, collect lab specimens, assist with routine tests, prepare and clean exam rooms and order supplies and equipment.  CMA will be committed to creating a positive team environment while being a good team member.

Requirements/Qualifications
