Brief Summary
Under the direction of the Chief Medical Officer, Director of Nursing will oversee standards of nursing practice.  DON will coordinate clinical support services required for timely and effective service delivery to patients in a Patient-Centered Medical Home (PCMH) model of care.  Responsible for knowledge of and compliance, with regulatory or credentialing agencies, such as Joint Commission, state and federal agencies.

Requirements/Qualifications
Bachelor’s Degree in Nursing required or 3-5 years clinical management experience, preferably in ambulatory setting. Current NJ RN licensure & BLS certification.

Brief Summary
Reporting to the Chief Medical Officer, The Clinical Director is responsible for assuring the delivery of comprehensive, quality health care and productivity that is appropriate for the patient and CAMcare. The Director will recruit and retain competent, qualified providers who share our mission. Responsible for the oversight of all clinical personnel in their discipline and creates an atmosphere of team spirit and cooperation among staff members and patients. Aids in the coordination of related grant, educational activities and primary care research done at the Center. Facilitates the continuity of care between patients and external providers. Participates in business development and outreach efforts.

Requirements/Qualifications
MD or DO degree from an accredited School of Medicine. Active NJ Licensure, Board Certified in OB/GYN. Active NJ CDS and DEA licensure and current BLS certification. Maintenance of Continuing Education requirements for licensure and skills maintenance. Ability to relate and communicate well with diverse population.


Licensed Clinical Social Worker (LCSW)

Brief Summary
LCSW has the primary responsibility for providing behavioral mental health and social services under the direction of the Clinical Supervisor of Behavioral Health, Mental Health and Social Services. In accordance with policies, procedures, and protocols established by; standards of clinical social work practice and NJ licensing and certification, and other regulatory agency requirements as appropriate. Responsible for implementing community educational programs and determining appropriate types and methods of treatment, and implementing intervention methodologies; acting as program consultant to staff members; functioning in such areas as counseling, protective services and family services.

Requirements/Qualifications
Master’s Degree in Social Work (MSW). Current State of NJ Licensed Clinical Social Worker License. At least three years post graduate social work experience. Strong interpersonal, communication and organizational skills. Proficient in MS Office and EMR software. Ability to relate and communicate well with diverse population. Bilingual (English/Spanish) a plus.

The Nurse Manager schedules, assigns, and reviews work of all ancillary staff, provide staff training, conducts performance evaluations, provides direct patient care when needed and responds to medical emergencies. Other duties include participation in Quality Assurance and Quality Improvement activities, making recommendations on departmental Policy and Procedures and collaboration with the Director of Nursing to ensure unit compliance with New Jersey State Department of Health, PCMH and JCAHO requirements. The nurse manager attends monthly meetings for nursing, departmental and management teams, and all other duties as assigned by the VP/Director of Nursing.

Requirements/Qualifications
2- 4 years experience as Nurse Manager. Ability to communicate and collaborate effectively with a variety of professionals, ancillary, and administrative personnel. Strong interpersonal, communication, and organizational skills. Associates Degree in related field required.
Bachelors of Science (RN’s) preferred or other related bachelors degree.
3 – 5 years nursing experience in medical center or community health care settings
Active State of New Jersey RN license required. Basic Certified Life Support is required.

OB/GYN RN Case Manager

Graduate from an accredited health services institute. Experience or graduate from accredited Dental Assisting program with externship program.HS graduate or equivalent required.(CDA) Certified Dental Assistant from accredited program. Brief Summary
Under the supervision of the Care Coordination RN and Primary Care Director, the Care Management LPN will be responsible for the clinical case management of and outreach to patients that are a part of CAMcare’s Medical Home Savings Program.  They will develop outcomes based strategies to proactively engage UHC, Horizon, & other CAMcare patients as assigned.  The Care Management LPN will collaborate with health specialists to determine patient health needs and the availability of services, and to develop goals & action plans for meeting those needs.  

Requirements/Qualifications
Graduation from an accredited LPN program with two or more years of hospital or public health nursing experience.  Current and valid NJ Practical Nurse licensure and BLS certification.  At least one year of experience in each of the following disciplines; Internal Medicine, Pediatrics or OB/GYN.  Daily access to reliable transportation for home, hospital, and/or other community service visits. Proficient in MS Office and EMR software. Ability to relate and communicate well with diverse population.  Bilingual (English/Spanish) a plus.


Certified Medical Assistant

Brief Summary
Under the direction of RN, the CMA will room patients, take vital signs, collect lab specimens, assist with routine tests, prepare and clean exam rooms and order supplies and equipment.  CMA will be committed to creating a positive team environment while being a good team member.
